Take the Health Assessment and Earn $30
How It Works
The 20-minute health assessment asks about your diet, exercise and preventive care routines. You’ll find it through MyActiveHealth (formerly ActivePHR), which stores, tracks and organizes your health information. When you finish the assessment, you’ll receive a health report to help you better understand your health status and your potential risk for conditions such as diabetes or heart disease. All adult enrollees, including spouses and dependents 18 and older, can take the health assessment and get the $30 gift card.
How to Earn the $30 Bonus Award
Health Counts includes $30 gift card bonus awards for members who make healthy choices, on top of the reward for taking the health assessment. Here’s how you can earn a $30 bonus:
- You have diabetes or a heart condition and you participate in the Informed Care Management program.
- You smoke or chew tobacco and you participate in the Quit For Life tobacco cessation program.
- You exercise enough to meet the federal government’s minimum physical activity guidelines for adults and you report your exercise habits when you take the health assessment.
You can earn only one $30 bonus reward, in addition to the $30 gift card for doing the health assessment. If you qualify for the Informed Care Management or Quit For Life programs, you need to participate in the recommended program to earn the bonus and you cannot receive the bonus for exercising, even if you qualify.
Redeeming Your Gift Cards
You’ll receive your $30 gift card by email from Hallmark Insights* between five and 10 days after you complete the assessment. If your gift card doesn't arrive within that time, check your junk mail or spam folders. To make sure your card arrives in your email inbox, add HallmarkInsights@hallmarkinsights.com to your email contact list. Redeem your card by clicking on the link in the email. Bonus gift cards may take longer to process.
Your Privacy is Protected
Information you enter into your health assessment, as well as the data stored in your MyActiveHealth account, are protected. The health assessment and MyActiveHealth are provided by ActiveHealth Management, a third-party vendor that adheres to privacy laws, so no one else has access to your personal data or results.
