How It Works
ActivePHR keeps track of all of your doctor visits, health screenings and prescription drug history. The information is automatically added every time you receive services through your health plan. You can add information that only you would know, like any over-the-counter medications that you take.

• Lets you print out information, like a list of your child’s immunizations or an emergency wallet card.
• Lets you create email reminders about preventive care screenings or other exams and tests.
• Makes it easy to share information with your doctors. You can print out a health summary for your doctor – or even let your doctor see your ActivePHR online.
• Compares your health information against the latest medical knowledge. If it finds anything you should know about, it sends you an alert so you can use it to talk with your doctor.
